PARENTAL CONSENT & LIABILITY WAIVER
By electronically signing this agreement, I give permission for my child (named above) to participate in all activities of the Flight Club 502 Aviation Camp, including educational tours and hands-on aviation activities.
I acknowledge and agree that:
• The Louisville Regional Airport Authority (LRAA), Flight Club 502, and its partners are not liable for any unforeseen circumstances that may affect camp programming.
• Secure areas of the airport may require mature behavior. Flight Club 502 maintains a zero-tolerance policy regarding inappropriate conduct.
• Ground transportation for offsite activities may be provided by adult volunteers and senior youth leaders.
By signing below, I agree to release Flight Club 502, LRAA, and their respective officers, agents, and employees from any liability related to my child’s participation in the program, except in cases of willful misconduct.
PHOTO CONSENT
Flight Club 502 requires permission to use your child’s photograph to promote aviation camps and Flight Club 502 programs. Images may be used in print and online publications, presentations, websites, and social media. No royalty, fee, or compensation will be provided for the use of these images.
The recommended contribution for Fridays at Flight Club 502 is $75.
